    Preparing for vaginoplasty: Pre-Surgery Guide

    Choosing to undergo vaginoplasty is one of the most important and life-affirming decisions many trans women and transfeminine people will ever make. It’s not just a medical process, it’s an emotional and personal journey that requires preparation, support, and trust.

    Based on the experiences of former patients and the recommendations of our team, here is a guide to help you feel confident and supported before, during, and after surgery.

    1- Emotional and Practical Support From the Start with Safe Rebirth.

    Your First Point of Contact: The Patient Advocate

    Before even meeting with surgeons, you’ll be able to speak directly with your patient advocate. She is available by WhatsApp, phone call, or your preferred method to answer any questions you may have before surgery. This first connection is designed to give you peace of mind, free from pressure, no concern is too small.

    2- Free Video Consultations With Surgeons.

    Even if you already have a preferred surgeon, speaking with two doctors may give you a broader perspective and helps you feel fully secure in your decision. If you feel you need a second consultation or third with the same surgeon, that is also possible at no extra cost.

    To schedule your video consultations with the surgeons, your patient advocate will kindly ask for:

    • Your full name and preferred name
    • Your date of birth
    • Your height and weight
    • Your email and WhatsApp number
    • Front and side pictures of the area to be treated

    These details allow the surgeons to evaluate your case and discuss the most appropriate surgical technique for you.

    3- Preparing Logistically and Medically

    Once your surgery is confirmed, you’ll begin preparing for the practical aspects:

    • Pre-op tests usually include ECG, chest X-ray, and a full blood panel. Pre-op prices in some countries (e.g. the UK) can reach around £1,000. If you have a good relationship with your GP, it may be more cost-effective to do them there. We encourage patients to compare prices and practicality before booking.
    • Around two months before your surgery, you’ll receive a detailed preoperative guideline that covers preparation, surgery, and the recovery period. This document is clear, informative, and will be your roadmap throughout the process.
    • About a week before surgery, you’ll sign the consent forms. These explain the procedure, risks, and potential complications so that you are fully informed.

    Alongside the pre-op instructions, you’ll also receive a personalized travel plan with accommodation options close to the clinic. Many patients appreciate staying within walking distance for peace of mind.

    Where to do the pre-op tests?

    • We recommend doing them in your home country, so that if anything needs correction, you have time to resolve it before traveling.
    • If you prefer to do them in Spain, they can be performed at the clinic or hospital, depending on the surgeon, for around €150–€250 in total. We recommend making sure your doctor tests PT and APTT (blood coagulation is crucial before surgery).
    • Patients traveling from outside Europe (for example, from the U.S., Canada, or Australia) may benefit from doing their pre-op in Barcelona. In this case, we suggest arriving 4 days before surgery to optimize your time.
      • Around two weeks before your surgery, you’ll receive a detailed preoperative guideline (also known as The Vaginoplasty Booklet) that covers preparation, surgery, and the recovery period. This document is clear, informative, and will be your roadmap throughout the process.

    4- The Importance of Mental and Physical Readiness

    Just as important as your physical preparation is your mental readiness. This surgery is a deeply personal step, and making sure you feel calm, certain, and supported is essential.

    You’ll also be asked to follow specific instructions, such as stopping estrogen temporarily, quitting smoking, and avoiding alcohol before and after surgery. These steps might feel small, but they make a big difference for both surgery safety and recovery.

    5- At the Clinic: Care and Comfort

    When you arrive at the clinic, you’ll feel welcomed and cared for. Many patients compare the environment to a hotel rather than a hospital, which helps reduce stress. The staff are attentive, compassionate, and available at all times.

    During your stay:

    • You’ll be supported with pain management, antibiotics, and careful monitoring.
    • You’ll receive not only a dilation kit but also a medication kit, so you don’t need to go to the pharmacy yourself—especially helpful if you’re traveling alone. Both kits are included at no extra cost.
    • You’ll be given a vaginoplasty booklet with integrated menus and a low-residue diet plan (lots of chicken, easy to digest). Patients should expect to lose a little weight during this diet, which helps prepare the body for healing.
    • You’ll gradually resume drinking, eating, moving, and walking.

    Family members or friends are welcome to join you, and accommodations for them can often be arranged as part of your stay.

    6- You are never alone : Dilations and Ongoing Support

    On the day of your discharge, you’ll usually have your first dilation session with guidance from the care team.
    • If, for any reason, you miss your first dilation session, don’t worry. A gender nurse will personally guide you on how to dilate properly, free of charge.

    • If a dilator is lost or damaged, we will send a replacement directly to your home country.

    • All dilation support and follow-up care are included, with no additional costs.

    You’ll also have direct contact numbers for peace of mind:

    • Your personal gender nurse’s phone for quick questions or guidance.
    • Your surgeon’s direct phone, so you can feel secure even outside scheduled appointments.

    • A dedicated emergency line, available at all times.

    • Your patient advocate’s phone, to help with any practical or emotional support you may need.

    This system ensures you are never left alone and always have immediate access to someone who can help.

    7- The First Weeks After Surgery

    The first days focus on rest and gentle recovery. Swelling and soreness are normal, but many patients report feeling unexpectedly clear-headed and even joyful. Seeing the results for the first time is often described as a powerful, life-changing moment.

    Once discharged from the clinic, you’ll continue recovery nearby, usually for two weeks — with quick access to your surgical team if needed.

    8- Follow-Up Care

    Healing doesn’t end when you leave the clinic — follow-up appointments are a cornerstone of your care.

    You’ll have regular video calls with your surgeon in the weeks and months after surgery. These sessions give you the chance to ask questions, share your progress, and make sure everything is healing as expected.

    Dilation is also a key part of recovery. You’ll receive clear instructions, the right tools, and ongoing support. While many patients feel nervous at first, over time dilation becomes a routine part of daily life and far less intimidating.

    You will always have access to your care team by email or phone. Post-op support is included at no additional cost and will always be provided in a timely manner.

    9- Financial and Administrative Support

    If you need help from your surgeon, for reimbursement purposes or Doctor´s notes, this can be provided and arranged by your advocate, simply request it. Our team is also available to assist with any administrative questions along the way.
