Safe Rebirth Patient Agreement – Content & Discount Clause

The patient acknowledges that any reduction in the total price of services provided through Safe Rebirth has been granted conditionally, based on the patient’s explicit agreement to contribute specific materials that may include, but are not limited to: written testimonials (public or anonymized), before and after photos of surgical results and one or more video testimonials about their experience.
These materials are intended to help other future patients make informed decisions and to support Safe Rebirth’s mission of increasing access to transparent, real-life experiences in gender-affirming surgeries.
He patient agrees to deliver these materials as required with the patient advocate and post-operative pictures within 60 days following the date of surgery, unless otherwise arranged in writing.
If the patient fails to provide the agreed-upon content by that time without a valid reason communicated in advance, Safe Rebirth reserves the right to reclaim the equivalent amount of the discount applied to the total cost of their surgery coordination service. This amount shall be paid by the patient upon request via bank transfer or any other agreed payment method.
This clause is binding, and by signing below, the patient acknowledges their understanding of and agreement with these terms.
Medical Disclaimer and Waiver of Liability
1. Nature of the service
Safe Rebirth is not a clinic or a medical provider. We help you explore surgical options, we connect you with independent surgeons and clinics, and support your decision-making process. Safe Rebirth directly does not provide medical care or perform any procedures.
2. Independent medical providers
All medical care — including consultations, assessments, surgeries, and follow-up — is the sole responsibility of the surgeon or clinic you choose. The medical relationship is established directly between you and the healthcare professional.
3. No medical liability from Safe Rebirth
Safe Rebirth is not responsible for the medical decisions made by third parties, surgical outcomes, side effects, medical complications or any dissatisfaction related to treatment or medical care.
Any complaints, claims, or legal issues related to the surgery must be addressed directly with the surgeon or clinic.
4. Voluntary and informed decision
You are making your own informed and voluntary decision regarding which surgeon to choose. Our role is to provide information and support, but we do not influence medical decisions or guarantee outcomes.
